4 Steps to Living Well
Getting help for your loved one starts by discussing your options with an ElderCare Solutions of Michigan Care Manager. ElderCare Solutions of Michigan has devised our “4 Steps to Better Living” process. Each step is designed to minimize cost and maximize the benefits to ensure the best care possible.
Step 1: Comprehensive Assessment
This in-person, comprehensive assessment is designed to optimize a senior’s ability to enjoy good health, improve their overall quality of life, reduce the need for hospitalization and enable them to live as independently as possible.

Once this information is gathered, our Care Managers are able to address the client’s immediate needs. Through ongoing Care Management services, we continue to offer assistance as those needs change over time.

Step 2: Sharing a Care Plan
The care plan includes the results of the in-person assessment. We discuss options, provide recommendations and share ideas and referrals. We go into great depth explaining:
- What led to the recommendations
- What you can expect
- A prioritized needs list
- Costs of services
Step 3: Implement Care Plan
After the care plan has been shared and the Care Manager and family decide what steps to take, we have a discussion about what care the family feels they are able to do on their own and what they would like ElderCare Solutions of Michigan to handle for them. Our service is customized to fit each client and family’s best interests. To provide peace of mind, we closely keep an eye on every detail to ensure that proper arrangements for service delivery are addressed.
Step 4: Gaining Peace of Mind
With our ongoing support services, you gain peace of mind knowing that we watch our client’s progress, adjusting the level of services as needed, making certain that their best interests, safety and happiness come first. You choose the menu of services you want, and you’ll receive regular follow-up and communication. This can be provided monthly or more frequently depending on the circumstances.
