1. Comprehensive Assessment
This thorough assessment includes reviewing the older adult's:

2. Family Consultation
Sometimes family members prefer to meet with a Care Manager without an older adult present. A family consultation allows you the privacy to ask questions, address concerns and receive advice, in order to develop a plan of action.
3. Crisis Management
If you are concerned about an older adult who is in the hospital or experiencing a crisis, and needs assistance immediately, our Care Managers can often help with a same day appointment, offering advocacy, assistance and emotional support. Click here to learn more.
