What is Geriatric Care Management?
Geriatric Care Management is the process of caring for and coordinating the needs of older adults. At ElderCare Solutions of Michigan, our goal is to promote good health, improve overall quality of life, reduce the need for hospitalization, all the while working to enable independent living for as long as possible.
Each client of ElderCare Solutions of Michigan is assigned a Care Manager who helps not only with everyday needs, but also keeps a watchful eye on a monthly basis, standing by 24/7 in the event of an emergency. Our Care Managers are health and human service professionals who act as a General Contractor for your well-being. Think of us as an extension of your own family, looking out for your best interests. They are knowledgeable, warm and compassionate people who coordinate, observe, and adjust the level of services our clients receive.
